Body Contouring
Liposuction
Despite good health, regular exercise, and a healthy diet, some patients have disproportionate contours related to isolated fat deposits in the body or face. Removing these specific fatty deposits can result in improved body proportions, proper fit of clothing, and enhanced self-esteem. Liposuction, the most common plastic surgical procedure, can be effectively used to improve contour of the thighs, hips buttocks, abdomen waist, upper arms, back, inner knees, chest area, cheeks, chin, neck, and calves and ankles. It may take several months before the final results of liposuction are apparent.

Liposculpture
In contrast, liposculpture uses traditional liposuction techniques to treat multiple areas in order to create a harmonious overall body contour and may involve the calves, thighs, torso, and arms. This is often useful for overweight patients. The goal is to reshape, not to lose weight.

Autologous Fat injection
When performing liposuction, your own fat can be harvested, purified and re-injected to augment other areas of the body including the face, breasts, and buttocks. Multiple surgical sessions may be required to safely achieve the desired result.

Abdominoplasty (Tummy tuck)
Pregnancy, aging, weight fluctuations, heredity, and prior surgery may render the abdominal tissues loose and sagging despite our best efforts to exercise and eat right! For many patients, the best option is to undergo an abdominoplasty, or tummy tuck, to correct these problems. The surgical goals of abdominoplasty are to remove excess fat and skin, tighten weakened or separated muscles to create a smoother, flatter contour. This is not a treatment for stretch marks, per se, but some stretch marks in the lower abdomen will often be excised during the procedure. Upper abdominal stretch marks typically remain. Whether you are a candidate for a mini-abdominoplasty depends on the amount of extra skin you have in the area above the belly button. Abdominoplasty is sometimes combined with liposuction to specific areas, sometimes referred to as a Lipo-abdominoplasty.

“Mommy Makeover”
After pregnancy, the breasts and abdominal tissues are stretched and can leave a woman with drooping breasts and volume loss as well as a bulging or misshapen anterior abdomen. Many women choose to undergo reshaping of both the breasts and abdomen at the same time, choosing to have a breast lift with or without augmentation, and a tummy tuck in the same operation.

Arm Lift (Brachioplasty)
Aging, heredity and weight loss can cause upper arm tissues to droop and sag due to a loss of elasticity. While exercise will improve the underlying muscle tone and form, it will not tighten the skin and it often does not address those insidious localized fatty deposits. Often, patients are limited with respect to clothing choice and this can be a motivating factor in deciding to have an arm lift. By a combination of surgical excision and liposuction, we can reduce excess skin and fat resulting in a smoother, more toned appearance. The trade off for this improved appearance is a scar on the inside of the upper arms. Generally, if you have excess skin or looseness extending from the armpit to the elbow, then you will require a corresponding long scar. If the skin excess is localized to the area adjacent to the armpit only, you may be a candidate for a short-scar brachioplasty.

Thigh Lift
Many procedures resulting in a variety of scar patterns have been designed to treat thigh looseness. In general, the specific distribution of skin laxity and your aesthetic goals will determine which technique is best for you. If the laxity is limited to the upper inner and anterior thigh, then you may be a candidate for a relatively hidden scar in the groin crease. If however, the laxity extends to the area above the inner knee, then you may require an incision that runs the length of your inner thigh. Laxity of the lateral side of the thighs and buttocks is best addressed with an incision located at the top of your buttocks and hip bones. If you have excess fat and skin, then a combination procedure or multiple procedures involving liposuction and surgical excision (lipectomy) may be recommended.

Body Contouring after Major Weight loss
Following gastric bypass or any other substantial weight loss, the severely stretched skin and underlying tissues do not contour to fit the reduced core size. This can result in a generalized looseness and sagging of the body tissues from the face and neck to the knees. For many patients, surgical correction of this laxity is the final step in the weight loss journey. For long term results, it is important that your body wight reach a plateau for 6 months prior to surgery and that you maintain this healthy weight after any surgery.
Dr. Hertl is an established expert in body contouring. She has 15 years of experience achieving excellent results performing complex procedures for weight loss patients. She has taught her techniques to dozens of residents in Europe and at the Harvard Combined Plastic Surgery Residency. These operations require careful analysis, planning, and a demonstrated knowledge-base of special techniques in order to achieve excellent results. Understandably, most patients want to have many areas treated during one surgical procedure, but after extreme weight loss, patients may require multiple surgeries. Dr. Hertl’s proven philosophy is to meet the patient’s objectives, whenever possible, while always maintaining patient safety and avoiding complications. Trust your body to someone with proven experience.

Body Lift
Total body skin laxity is typically divided into 2 parts addressing the upper and the lower body as distinct areas. The lower body lift is a very powerful technique which treats circumferential laxity of the lower torso. Specifically, sagging of the abdomen, groin areas, buttocks, and upper thighs is addressed in a single operation.
The upper body lift procedure addresses laxity of the arms, breasts and upper back in one procedure. If weight loss has resulted in a loss of breast volume, then the tissues of the upper back can be rotated to the front to provide breast augmentation with your own tissues (intercostal perforator flap). These are complicated procedures tailored individually to the specific needs of each patient. Dr. Hertl has extensive experience in this area.
