  • Botox® and Facial Fillers
                     Botox® and Facial Fillers
All injections are performed by Dr. Hertl

As we age the tissues of the face descend and we lose volume resulting in wrinkles and loose skin. Fillers are an excellent way to restore volume and stimulate collagen growth. Often as we age the corners of the mouth droop blending with the marionette lines and creating a very-aged appearance.  Lidocaine is mixed with the filler to reduce discomfort and a topical anesthetic cream is applied to further ease the application process. Complications related to injection therapy are usually limited to redness at the injection site, bruising, and temporary swelling. Arnica Montana tablets and cream are very effective in limiting the duration of this effect when it occurs.  These injectable medications are different in their formulations and clinical effect and are most effective when used together in a complimentary way. For example, Botox treats dynamic wrinkles while fillers are used to treat static lines and volume deficiencies. Further, the use of Botox can extend the duration of filler effect.

BOTOX® Cosmetic

Botox cosmetic is a prescription medication called a neuromodulator (Onobotulinum toxin A) that is injected into the muscles of your face to temporarily treat moderate to severe frown lines. It works by blocking nerve impulses to the injected muscles. This reduces muscle activity that causes lines to form. Botox can be used to reduce your 11s between the brows, crow’s feet at the corner of your eyes, peri-oral lines and when applied to the lateral aspect below the lateral aspect of your eyebrow can result in a significant brow lift effect.

Treatment with Botox takes approximately 10 minutes. Discomfort is usually minimal and brief; however, an anesthetic cream can be applied 30 minutes prior to your injection should you prefer. The results are seen in 24-48 hours and you may notice continued improvement over the next few weeks. Botox lasts 3 to 4 months depending on the individual and best results are achieved when Botox is re-injected on a regular schedule before the effects fully wear off.

Juvéderm® Injectable

Juvederm is a second-generation hyaluronic acid which is a naturally hydrating substance that is commonly used to smooth away facial wrinkles, nasolabial folds, and mild facial volume loss. It is very effective for filling fine lines and is the treatment of choice for lip augmentation. The effects of Juvederm are immediate, but continue to improve over the first 1-2 months. Results last  approximately one year but the results are dependent on the individual.  Juvederm is formulated with the local anesthetic lidocaine, to provide a more comfortable experience during and after treatment.

Radiesse®

Radiesse® Volumizing Filler, is a filler that contains calcium hydroxyapatite in a gel carrier. It can be used to treat moderate to severe facial wrinkles and folds and is most effective in restoring volume to the cheek and peri-oral areas. Smile lines, marionette lines and naso-labial folds can be eliminated. Radiesse can be applied in the pre-jowl sulcus to camouflage the appearance of jowls and improve the jawline contour. It is very effective in elevating the corners of the mouth as well. It is clinically proven to last a year or more.

Autologous Fat Injection

Your own fat can be harvested in the office or in the operating room at the time of other surgery and then re-injected as a filler. Fat is an excellent facial filler , excluding the lips where it rapidly decreases in volume. A hyaluronic acid filler, such as Juvederm, is the preferred choice for the lips. (Fat can also be used in the breasts or buttock areas for augmentation.)

Liquid facelift

For patients wishing to avoid surgical procedures for facial rejuvenation, the combination of injectable  Botox and fillers can be used to temporarily achieve remarkable and satisfying results. Each face is different, so the goal is to globally treat the many aspects of facial aging in an individual way. Each patient is carefully assessed and a personal care plan devised.

In the upper face, we can use
Botox to eliminate dynamic wrinkles of the brow, between the eye’s (glabella), and crow’s feet at the corners of the eyes. Botox can also be applied to the lateral brow to achieve a browlift effect. Fillers such as Restylane and Juvaderm can be used to camouflage under eye circles and thinning of the temporal areas.

The midface can be treated with fillers, Radiesse and Juvaderm , to correct the cheek  volume loss and midface descent. This, in turn, allows us to use less filler in the nasolabial fold area (groove extending from the corner of your nose the to the corner of the mouth).

As we age, the lips undergo a significant masculinizing change with loss of volume, loss of the defining border (white roll), and the development of both static and dynamic fine lines and wrinkles. These changes are best addressed with injectables. Small amounts of botox can be injected at the borders of the lips to relax radial lines in this area. Additionally, Juvaderm can be added to treat residual fine lines and to augment the lips themselves. Patients are often surprised and delighted by the positive change this treatment achieves.

In the lower face, the aging process results in the formation of jowls at the jawline, marionette lines extending form the corners of the mouth, and descent of the corners of the mouth causing a frowning appearance. Banding of the platysma  muscles and vertical lines may appear in the neck area.  Botox and fillers in combination can be used to fill the marionette lines and elevate the corners of the mouth. Fillers can be applied to the area in front of and behind the jowls to smooth the jawline. Botox can be injected into the platysmal bands to relax them, and filler can be applied to vertical neck lines.
